Provider Toolkit. The 2008 Providers Immunization Toolkit for Children & Adults is a
resource guide containing the most current standards of practice, forms,
& vaccination methodologies for immunization in Michigan. If you are not
able to find the information you need (other than specific questions),
please describe what you need in the "survey" section, or click on
"Contact Us".
Specific immunization questions should be directed to the National
Immunization Program at the Centers for Disease Control website.
The Alliance for Immunization in Michigan (AIM) is a non-profit,
all-volunteer partnership of healthcare professionals & agencies that
provide & promote immunizations across the human life span. AIM would
